Small changes in blood flow or oxygen uptake seen in some studies don’t translate into meaningful gains in endurance. Under special conditions (e.g., high altitude): Some studies suggest sildenafil may slightly affect pulmonary circulation at high altitude (by lowering pulmonary artery pressure), which could modestly alter oxygen saturation but still does not reliably improve endurance performance. Animal research vs. humans: Experimental studies in animals showed sildenafil might affect peripheral markers of fatigue and time to exhaustion in controlled settings. However, such results don’t automatically apply to humans, and clinical evidence for real?world exercise performance benefit in people is limited and inconsistent.
