Filitra is a PDE-5 inhibitor. Its main action is on the smooth muscle of blood vessels in the penis, where it increases the effect of nitric oxide (NO)–cGMP signaling: Sexual stimulation causes release of nitric oxide in penile tissue. Nitric oxide increases cGMP, which relaxes smooth muscle. Relaxed penile blood vessels widen (vasodilation), allowing more blood flow and helping produce an erection. Vardenafil blocks Filitra , the enzyme that breaks down cGMP, so the relaxation effect lasts longer. Because PDE-5 is also present in some systemic blood vessels, Filitra can cause mild blood vessel relaxation throughout the body, which may lead to: Flushing (warmth or redness of the face) Headache Dizziness A small drop in blood pressure This effect is usually mild in healthy people, but it can become risky when combined with nitrate medicines (such as some drugs for chest pain), because blood pressure can fall dangerously low.
