Can an MBBS doctor pursue a Fellowship in Emergency Medicine after completing internship, or is postgraduate training required?

I’m looking into emergency medicine as a career option and want to understand the available training pathways in India. Emergency departments handle a wide range of acute conditions, so I assume additional training could be useful for doctors who want to build confidence in assessing and stabilizing patients.

Some fellowship programmes may accept MBBS graduates, while others may have specific requirements related to experience, registration or prior clinical work. Eligibility can vary between institutions, so it is important to read the admission criteria carefully instead of assuming every course has the same requirements.

For an MBBS doctor, useful topics in a fellowship may include triage, initial patient assessment, airway and breathing emergencies, shock, trauma, cardiac emergencies, poisoning, acute neurological presentations and emergency procedures. The amount of practical exposure matters as much as the syllabus. I would specifically ask whether hospital training is included, how it is supervised and what responsibilities trainees are permitted to undertake.

Another point I’m trying to clarify is the difference between completing a fellowship and becoming a formally recognized specialist. A fellowship may provide additional training, but it does not automatically replace an MD, DNB or other recognized postgraduate qualification.

If someone is considering this route after MBBS, I would suggest comparing programmes based on eligibility, clinical exposure, faculty, assessment, certificate-awarding body and the roles for which the training may be relevant.

Has anyone joined an Emergency Medicine fellowship directly after MBBS? Which parts of the programme were most useful, and what should a new graduate verify before enrolling?
